DAYTON, Ohio, Aug. 1, 2016 (GLOBE NEWSWIRE) — CareSource, a leading regional nonprofit health plan, has hired Jason Coons as the new Vice President for its corporate care management division, Care4U. Coons will lead the rollout of a care transformation model, Care4U, across all company markets and products.
Care4U is a dynamic, community-based, member-centric model of service delivery. The new model, designed as a population health management approach to care coordination for members, will promote health improvements for CareSource members.
An experienced leader in the health care industry, Coons has a background in leading strategic organizational change initiatives utilizing change management, strategic deployment, and analytics to turn strategy into business transformation. Previously Coons served as Network Director of Process Improvement at Kettering Health Network.

Launched in January of this year, the Care4U model provides CareSource members with access to a full-continuum of services with community-based, regional care coordination teams, positioned to enable members to access services that meet their individual needs.
Coons has a Bachelor of Science in Industrial Engineering from the University of Cincinnati and a Master of Health Administration from Ohio University.
