WEST CONSHOHOCKEN, Pa., Nov. 17, 2015 (GLOBE NEWSWIRE) — Tandigm Health and Doylestown Healthcare Partnership (DHP) today announced an innovative agreement to improve the delivery of quality health care, reduce costs, and enhance patient satisfaction. Under the agreement, DHP – a clinically-integrated network that includes Doylestown Health and primary care and specialist physicians – will collaborate with Tandigm Health and its extensive network of primary care physicians. Tandigm Health’s clinical expertise, analytic tools, and real-time data, combined with DHP’s capabilities in the management of patients with chronic illness can help doctors better manage difficult chronic conditions, expedite preventive care, and deliver more cost-effective, high-quality care.
The partnership marks Tandigm Health’s second cooperative arrangement this year with both a clinically-integrated network of primary care physicians and a health system providing a full continuum of health care services. This arrangement is Tandigm Health’s first that includes specialists, reinforcing the importance of hospitals, primary care physicians, and specialists working together to coordinate patient care. The agreement was effective October 1, 2015.
